The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community.
We are looking for the right people who can empower residents to take control of their lives. This will involve working with residents in their homes who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy, problem solving and enthusiasm. You will need excellent organisational skills, be adept at seeking out information for relevant community services and have the tenacity to help people achieve sustainable improvements in their wellbeing.
Main duties include:
· Managing a caseload of households.
· Providing monthly home visits and providing a continuous point of contact for all members of the household.
· Working with the residents to assess their needs, create personalised action plans and locate support within the community.
· Supporting residents to prioritise and address practical, health, social and emotional issues in their lives which affect their wellbeing.

Carolyn House Service provides housing and support to individuals experiencing mental health issues for up to two years. We promote social inclusion and develop life skills for independent living. We also work with several agencies and organisations in the local area to meet individual customers' needs. We support customers to build confidence in order to empower them to use community resources; we promote healthy lifestyles and wellbeing and offer customers choice and control over the activities they would like to participate in.
The service is staffed 24 hours 7 days a week. All customers referred to the Service are allocated a support worker who will provide guidance and support on a range of issues and help signpost customers to relevant services located in the Redbridge Borough.
The Post holder will be expected to work on a rota basis (Monday - Sunday) and two sleep-ins per week which attracts £30.00 extra pay.The shift patterns are 8.00 am - 4.00pm, 2.00 pm - 10.00 pm and sleep ins.

Lester Court is a supported living service, serving the London Borough of Tower Hamlets. The service will supports seven adults with learning disabilities to thrive.
Specialist Support Workers within Look Ahead are required to have either an extra dimension of experience or a higher qualification level as determined relevant for the particular post. Specialist Support Workers provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. The Drive model is based on an intensive case management approach and explicitly targets high risk perpetrators of domestic abuse associated with victims who have been referred to the MARAC. About the role:
This role will be part of our IDVA team in the London Victim and Witness Service. As a DRIVE Independent Domestic Violence Advocate you will:
- Address the safety of victims at high risk of harm
- Serve as a victim's primary point of contact by working with their clients from the point of crisis to assess the level of risk, discuss the range of suitable options and develop safety plans.
- Be pro-active in implementing the plans, including actions from MARAC, which address immediate safety and enable victims to protect themselves and their children, as well as longer term solutions, using sanctions and services via criminal and civil courts, housing options and other organisations.
You will need:
- Have a knowledge of help-seeking barriers and support needs of victims of domestic abuse (in heterosexual or same-sex relationships).
- Have a good understanding of domestic abuse including the impact of domestic abuse on victims and their children.
- Understand the principles of risk assessment, safety planning and risk management for victims of domestic abuse and their children
- Understand safeguarding issues, and the legal responsibilities surrounding these issues.
- Direct service delivery to victims of domestic abuse or other vulnerable people.
- Have excellent communication, negotiation and advocacy skills, both written and verbal when interacting with a range of agencies and individuals. Understand and be committed to equal opportunities and diversity issues in policy and practice.
- Have a strong crisis management skills and the ability to deal with stressful and difficult situations.

Over 3000 children in the UK need adoption each year because they cannot remain at home or with family members. The CVAA Practice Programme delivers 30 or more online courses each year to those working in the sector, primarily social workers. Due to a restructure of the team, we are now looking for a Training Manager who can oversee the delivery of the programme from mid to late July 2024. Initially fixed term, it is anticipated that this post will become permanent.
